Managing Labor Pain Naturally: How Obstetric TENS Can Help

While there may be a few women who hardly feel any pain during labor, for most of us, childbirth comes with significant pain. Thanks to modern medicine, epidural anesthesia is available in every hospital in Belgium, providing the most effective relief from labor pain. However, epidurals come with potential side effects and risks, which makes exploring less medicalized, natural pain relief methods worthwhile. One such option is the TENS machine, and in this post, I’ll cover everything you need to know about it—including where to find one, how much it might cost, and a special opportunity for my clients.

What Is TENS?

TENS stands for Transcutaneous Electrical Nerve Stimulation. You might have come across TENS if you’ve ever had physiotherapy for a sports injury or persistent back or shoulder pain. A typical TENS unit consists of a central control device and adhesive electrode pads that are placed on the skin in the area of discomfort. These devices use electrical impulses to help reduce pain.

Conventional TENS units have various programs that adjust the frequency and rhythm of the impulses depending on the type of pain being treated. The intensity of the impulses can also be controlled, with lower levels producing a gentle tingling sensation, while higher levels may feel quite strong. It’s important to note that TENS does not treat the cause of pain—it simply alleviates the sensation of pain. This makes it a great tool during labor, where managing pain is a key need.

How Does TENS Work?

The pain-relieving effect of TENS is similar to what happens when you bump your elbow and instinctively press your hand against it to ease the discomfort. This creates a counter-stimulation that occupies the nerve pathways transmitting pain signals to the brain, effectively slowing down or blocking the sensation of pain. Additionally, the rhythmic, low-intensity impulses from TENS can stimulate the production of endorphins—your body’s natural painkillers.

Using TENS for Labor

Although standard TENS machines are readily available for home use, they are not ideal for labor because labor pain comes in waves, requiring the electrical impulses to be adjusted accordingly. This is where obstetric TENS devices shine. Designed specifically for labor, these units feature a burst mode for rhythmic pulses that promote endorphin release and a boost mode for continuous stimulation during contractions. The boost mode is activated by a button on the device, allowing you to control it easily, even with your eyes closed.

Advantages of Obstetric TENS:

  1. No Side Effects: Safe for both mother and baby, TENS is considered a "natural" pain relief method.

  2. Control: You control the intensity and mode, fostering independence and reducing feelings of vulnerability.

  3. Ease of Use: Obstetric TENS devices are simple to operate, even during intense moments.

  4. Continuous Use: You can start using TENS early in labor and continue through to the pushing stage, gradually increasing the intensity as needed.

Considerations and Limitations:

  • Limited Awareness: TENS for labor is not widely recognized in Belgium. Some medical staff may be unfamiliar with its use.

  • CTG Compatibility: While some may worry about interference with fetal heart rate monitoring (CTG), this is typically not an issue. If concerns arise, we can test the CTG with and without the TENS on.

  • Water Use: TENS cannot be used in water (e.g., birthing pools or showers).

  • Exclusions: It’s unsuitable for use over broken skin or for individuals with pacemakers, epilepsy, or a high stroke risk.

Choosing an Obstetric TENS Device

Popular models include the Elle TENS (and the updated Elle TENS 2), which offer all the key features for labor, including a contraction timer in the newer version. Prices range from €100–€120 new, but second-hand options are often available for around €40. Another option is the TENSCare Perfect Mama, though it has received mixed reviews. Other brands, such as OBI TENS and Boots Maternity TENS, provide simpler, one-channel options at lower prices.

How to Use TENS During Labor

Place the electrode pads on your back in pairs: one pair just below the bra line and the other near the hips, on either side of the spine. It’s best to apply them early in labor to maximize their effectiveness. Replacement pads are available online for around €10–€12 if needed.

While it won’t provide the same level of relief as an epidural, many women find that it significantly reduces the intensity of contractions and gives them a greater sense of control.
